Pet Medication Order Form

If your pet has ongoing medication, please use the below form to request a repeat of the prescription. Please allow 2 business days for the medication to be made up. We will text you to let you know when your medication is ready. Your pet may require a consultation and health check prior to their medication being dispensed if it has been over 6 months since their last visit. In this instance, we will call you to let you know if the veterinarian requests a check-up of your pet.
